DEVICE OFFERINGS

Except where a customer brings his or her own device to Telrite, handset models provided to Lifeline subscribers are selected at the sole discretion of Telrite. Telrite's devices comply with the Federal Communications Commission's ("FCC's") rules and are Wi-Fi-enabled. As required by FCC rules, some of Telrite's devices are hotspot enabled.

Telrite will provide subscribers with a smartphone at no cost to the Lifeline-eligible consumer.

Lost or stolen devices are subject to a $25 replacement fee.

To obtain Life Wireless service potential subscribers must meet certain eligibility requirements such as receiving governmental assistance or a household income that is 135% or below Federal Poverty guidelines for a household of that size, or the percentage guideline for your state. The specifics of what determines a potential subscriber's eligibility are specific to each state. Life Wireless service is limited to one per household, and cannot be combined with any other :label offering.
